Job Posting Home Health Aide - Home Health The Home Health Aid assist an RN and therapists in the care of the patient in providing personal nursing care on a scheduled basis for the sick and/or disabled member of the family. The duties of the Home Health Aide are planned and assigned by the RN to meet the patient and family situation. Observe patient's condition and report any unexpected change to the RN. Give safe, effective personal hygiene as planned by the RN or therapist. Maintain a safe, clean environment for the patient. Prepare and serve or have ready nutritional resources. Write reports such as list activities done, report observations and patient needs, and account for hours worked and mileage. Learn specific techniques from nursing and therapy personnel for use in providing aide service in the home. Participate in discussions and patient care as a member of the Health Team and attend the weekly Care Conference meeting. Functions independently and without direct supervision in the home.
